Do You Need a License for Reiki in Ohio?
Hands-off and distance energy healing needs no Ohio license, as long as you do not offer to diagnose or treat disease. Hands-on Reiki is riskier, because the State Medical Board says Reiki that manipulates soft tissue for a therapeutic purpose needs a massage therapy license.
Can I offer hands-on Reiki in Ohio?
This is a gray area that leans strict. Ohio defines massage therapy as treating disorders of the body by manipulating soft tissue, using techniques such as touch, stroking and kneading, and there is no exemption for Reiki or energy work. The State Medical Board's massage FAQ names reflexology, Reiki and polarity, and says a modality that manipulates soft tissue for a therapeutic purpose is massage therapy under any name and needs a massage therapy license.
The Board has not discussed light hand placement with no manipulation. If you touch clients, keep it to light, still hand placement and never call it massage, or get legal advice.

Can I offer hands-off or distance sessions?
Yes. Nothing in Ohio law or Board guidance addresses no-touch or distance work, and the massage definition requires manipulation of soft tissue, so work with no contact falls outside it. The medical practice act reaches anyone who, for compensation, examines, diagnoses, or prescribes or administers treatment for the cure or relief of injury, infirmity or disease. Do not offer to diagnose or treat illness.

Does it matter if I am paid?
Ohio law does not say clearly. The massage definition has no payment element, and the Board's FAQ makes no exception for unpaid work. The medical practice definition covers diagnosing or treating for compensation of any kind, direct or indirect. Treat free touch-based sessions with the same care as paid ones.

Which words and claims should I avoid?
Do not use Dr., Doctor, M.D. or physician in a medical sense, advertise that you practice medicine, or diagnose, prescribe or treat disease for compensation. Even licensed massage therapists may not diagnose in Ohio.
Ohio has no separate law protecting the word massage in advertising today. Leave it out anyway, since the Board treats Reiki with soft tissue manipulation as massage, and a 2026 bill, HB 744, would make it a misdemeanor to advertise massage that is not performed by a licensed or exempt person.

Do cities or counties have their own rules?
Yes. Townships may require registration of massage establishments and their workers, though they may not regulate state-licensed massage therapists. The Medical Board's FAQ says cities and townships may regulate non-therapeutic massage, each with its own rules. Check with your city, township and county.

How do I get a definite answer?
Ask the State Medical Board of Ohio in writing, and describe exactly how you use touch. Keep an eye on HB 744, which could change the massage definition before the legislative session ends on December 31, 2026. As introduced, it defines massage by technique, such as pressure, rubbing and tapping, and contains no exemption for energy work or light-touch bodywork.

Recent changes. HB 744, introduced March 10, 2026, would redefine massage by technique and contains no energy work exemption. As of October 31, 2026 it had only had a sponsor hearing, and Ohio's separate 2026 Interstate Massage Compact law, SB 276, does not affect energy work.
